Registration Form

Thank you for your interest in Dutch Fork Homeschool Resource Center! Please know thatDFHRCis a resource center and not a school. We are committed to assisting you and your family on your homeschooling journey by offering top-quality, Christ-centered instruction in subject areas in which parents/students need some assistance. Knowledgeable instructors teach material, provide assignments, and award grades for grade reports. However, you, the parent, are still responsible for the education of your child.Therefore, please maintain a high level of supervision over your student’s work and help us hold him/her accountable for the assignments we provide. From time to time we may request that you administer a take-home test or sign off on an assignment and/or document time spent on homework. It is imperative that all homework assignments are completed in a timely manner in order for us to provide the weekly instruction necessary for satisfactory course completion.

 LOCATION & MEETING DAY:Classes are held at Chapin Baptist Church, 950 Old Lexington Highway, Chapin, SC 29036. The majority of classes meet only on Mondays. Spanish courses meet Mondays and Fridays. Chemistry I meets Mondays and approximately every other Friday. (Instructor to provide meeting dates.)

IMPORTANT DATES: Deadline for registering without incurring a late fee is June 1st, 2019.

A mandatory orientation will be held the evening of August 12th, 2019. (Details forthcoming to registrants.) First semester runs from Aug. 19th – Dec. 9^th, 2019 and second semester runs from Jan. 6th, 2019 – April 27th, 2020. See our website at www.dutchforkhomeschoolresource.comfor a full class calendar.

FEES:

1. Registration/facility feeto be submitted with registration form and course matrix –

Non-refundable registration/facility fee of$100 for returningfamilies* and$115 for new families made payable toChapin Baptist Church for the use of their facilities.

1. Science lab and/or art supply feesto be submitted with Part 1(Registration Form)and Part 2 (Course Matrix)- Made payable to the appropriate instructors.(See Part 2for details.)

• Tuitionto be paid at orientation on August 12th,^, 2019for 1^stsemester and on January 6th, 2019for 2^ndsemester –$235 per semester, per class, per student made payable to individual instructors. Chemistry I is an additional $15 and all Spanish courses are an additional $25 since they meet more than once a week.

1. Late registration fee to be submitted with registration form, if necessary- If classroom space permits, registrations postmarked after the June 1st deadline may be accepted, however, a $25 late fee will be required if postmarked from June 1st – 30^th; $40 if postmarked from July 1st – 31^st; $55 if postmarked thereafter. Checks for late fees are to be made payable to Lisa Baghdady for use as a supply fee to cover additional needs for the center (i.e. website upkeep, online grade reporting, dry-erase markers, etc.).

 HOW TO REGISTER: Please print out and completePart 1(Registration Form) andPart 2(Course Matrix with selections circled. Submit one for each student.) and mail, along with all appropriate fees, to:

Your registration will be recorded, a confirmation e-mail will be sent to you and course instructor(s), and all checks will be delivered to the appropriate parties.

*“Returning families” are defined as those whose students attend DFHRC immediately prior to the year for which they are registering.
